Supporting Rural Communities With Tech-Enabled Services

Rainelle Medical Center is a community health center located in rural West Virginia. This Federally Qualified Health Center is also recognized by the National Committee for Quality Assurance as a Patient-Centered Medical Home (PCMH). This recognition distinguishes Rainelle as a practice that is continually committed to quality and patient-centered care. Rainelle is dedicated to providing high-quality comprehensive care to rural communities in West Virginia.

TimeDoc Health is supporting Rainelle to provide Chronic Care Management and Remote Patient Monitoring services to 4,000+ vulnerable patients with multiple chronic conditions.

Why We Love Working with Rainelle Medical Center

Rainelle recognized a need to support patients in their community, including the underserved, with holistic diabetes management initiatives. Their plans to kick off programs in support of these initiatives were stalled due to the onset of COVID-19. In December 2022, Rainelle was able to engage with TimeDoc Health in support of this initiative.

As a Federally Qualified Health Center, Rainelle qualifies for Center for Medicare and Medicaid Services (CMS) reimbursement programs centered around chronic care management, providing remote support to patients in between office visits. These programs are critical to helping health centers deliver the highest quality of care and to prioritize preventative care to patients regardless of their ability to pay.

The TimeDoc Health platform enables FQHC and Rural Health Centers like Rainelle to manage CCM and RPM programs, and track patient encounters, vitals, and other physiological data. We also provide clinically certified staff that acts on behalf of our provider partners to complete chronic care management and remote patient monitoring encounters. Our platforms integrate with EHR systems and provide all claims data for providers to easily submit for reimbursement.
