“During a recent care call, patient reported that he was referred to see a pulmonologist but was concerned about driving there due to pain in his feet. Patient also reported that he was feeling very overwhelmed. During the care call, CM realized that patient had difficulty reading his insurance card due to low literacy. CM worked with patient to contact his insurance provider to determine transportation benefits eligibility. CM was able to determine that patient is eligible for transportation assistance and $1,200 per year in over-the-counter benefits. While on the phone with the insurance provider, CM shared that patient has low literacy to ensure future communication from the insurance provider is able to accommodate him. CM educated patient on how to access these benefits, and he was successful in scheduling transportation for the pulmonology appointment on Monday.”
