Limestone Medical Center is partnering with TimeDoc Health and Texas A&M to enhance care for patients with longterm health conditions. This new program aims to support individuals in rural communities who have chronic illnesses such as diabetes, high blood pressure, heart failure, and lung disease.
Through TimeDoc Health, patients will receive additional support between doctor visits. Highly qualified TimeDoc care coordinators will check in with patients, monitor their health, and assist them in managing their treatment plans. With this partnership, TimeDoc collaborates with Limestone Medical Center’s doctors and nurses, enabling the hospital to closely track patients’ progress and make more informed decisions about their care.

This program makes it easier for patients to stay connected with their healthcare team—without always having to travel to a doctor’s office. It’s another way Limestone Medical Center is helping the community stay healthy.
