During a routine care call, a patient mentioned to our care manager that he couldn't take oxygen for over 48 hours because his concentrator was broken. He mentioned that his BiPAP machine was also not working properly, causing him to wake up feeling suffocated in the middle of the night.

Our care manager quickly arranged a temporary concentrator to be delivered to him within a few hours to start oxygen treatments immediately. She also worked with his primary care team to order him a new concentrator and BiPAP machine covered under his insurance, so he did not have to pay out of pocket.

Having a trusted relationship with his TimeDoc Health care manager, the patient called him to let him know the insulin his endocrinologist sent into the pharmacy was not there. Our care manager discovered the patient's speech was not clear during the call and asked him to test his blood glucose level, which ended up being higher than normal.

The care manager immediately got the endocrinology office on the line, which confirmed with the patient that he could take the 50/50 insulin he had on hand. In the meantime, his care manager set up automatic refills for delivery directly to the patient for no additional cost. The patient was extremely grateful for not having the constant worry of figuring out how to pick up his insulin and committed to checking his blood glucose levels every day to manage his own health better.

The patient disclosed to our care manager that she had mobility issues and could not make food for herself anymore. Through the monthly engagement call, the TimeDoc Health care manager discovered the patient lives by herself, and there is no one around to help her with meals.

The care manager was able to find and contact a local organization that supports the elderly and worked with the patient to fill out an application. She stayed in continuous contact with the patient until the application was approved. Not only was the patient able to receive meal delivery, but the TimeDoc Health care manager also worked with the organization to get her as an emergency response system at no cost since she lives alone.

When a TimeDoc Health care manager contacted a patient, she complained about being up all night with pain in her arms, chest, and back. Our care manager followed the health center's protocol and got her an appointment to come in the very next day.

After a successful procedure, she is recovering and stable and can avoid future health complications that could comprise the patient's health.

During their monthly engagement, the patient reported to the care manager that she had 20+ falls in her bathroom over the last 6 months but was embarrassed to tell her primary care provider since she could not afford the supplies needed to prevent this.

After the care manager notified the primary care physician, she found a medical supplier who could assist in getting the patient grab bars covered through insurance if she was able to provide proper documentation. The care manager worked with the physician's office to obtain paperwork and get the vital safety equipment within that month. The patient has reported a huge increase in quality of life since she no longer has to live in fear.
